Patient facing app/portal - ability for patients to input agreed health data with configurable symptom question sets and escalations. Data and information sharing - interoperability and process for pertinent data to be populated into the local Connected Care Record (CHIE). Ability to run / extract activity reporting. This has been awarded via a Mini-Competition through the SBS TECS2 framework,
