The programme aims to train and support local people and develop the capacity of services to improve health outcomes for communities and reduce health inequalities. Community Champions are local people who support their communities to improve their health and wellbeing. They communicate important health messages and make communities more aware of services and support to improve their health and wellbeing and how to access these. Community Champions work in partnership with health and care organisations. They collect and contribute local insight on a variety of health-related topics to help partners better understand local peoples health attitudes beliefs and behaviours. They collaborate with partners to co-design and deliver health interventions and inform the design or development of services. This will be for an initial three year contract with two options for annual extensions.
