The SPA will be for professionals only focusing on Adult Health and Social Care, including mental health and rapid response support, with signposting to all services including the voluntary sector. This project involves 5 partner originations working across South Tees, they include:- Redcar & Cleveland Borough Council, Middlesbrough Council, South Tees NHS Clinical Commissioning Group, South Tees NHS Foundation Trust and Tees Esk & Wear Valley NHS Foundation Trust. Phase one of the SPA is proposed to go live on the December 2016.
