The Service supports capacity for local authority social workers to make arrangements for both avoidance of an inappropriate admission to hospital and/or a permanent care home or for the transfer of care from hospital whilst further work is undertaken to complete arrangements for assessment of long-term care and support. The Service will be delivered through a residential care setting (a care home).
