The service will provide support for 'End of Life' (EOL) patients, as identified by the District Nursing Service, where a Continuing Health Care (CHC) package is yet to be put in place in place and/or where there are no family member or friends available to provide the necessary care. The Council proposes to enter a Contract for an initial period of 2 years with the successful tenderer (Service Provider), with no option to extend Contract Value: (£398,205.88 per annum) Providers are not required to submit any pricing schedule for this block contract.
